Zenoti's 2026 benchmark report puts the average medspa no-show rate at 4% and the cancellation rate at 14% for 2025, while broad healthcare research averages closer to 23% no-shows. You cut both with a deposit or card-on-file policy, a reminder sequence that asks for confirmation, and automation that refills and rebooks every empty slot.
You know the feeling. Your injector is set up, the room is prepped, and the 2 o'clock never walks in. We're an Orlando, Florida marketing team that builds booking, reminder and follow-up systems for medspas nationwide, and this is how we cut empty chairs without adding front desk hours.
For practices running a modern booking system, low single digits. Zenoti's 2026 Beauty and Wellness Benchmark Report, Medspa Edition, reports an average medspa no-show rate of 4% in 2025, down from 5% in 2024.
Across the wider beauty and wellness businesses it tracks, no-show rates ran from 1% to 4%.

Read that number with care. Zenoti is a booking software company, so its benchmarks describe practices already using a booking platform, most of which send automated reminders.
If you book by phone and confirm by memory, you're not starting from 4%.
Healthcare research shows what happens without that structure. A systematic literature review in Health Policy analyzed 105 studies on appointment no-shows and found an average rate of about 23%, ranging from 13.2% in Oceania to 43.0% in Africa.
Here's a quick way to read your own number. These bands are our working rule of thumb, not a published standard.
Zenoti reports an average medspa cancellation rate of 14% in 2025, down from 16% in 2024. That put medspas near the top of the 4% to 18% range across the wellness businesses it tracks.
So medspas lose far more appointments to cancellations than to no-shows.
A cancellation with a week of notice is easy to refill. A cancellation the morning of is nearly a no-show.
Track the two separately: total cancellations, and cancellations inside your policy window. The second number is the one that costs you.
Run the math with your own numbers. The example below uses illustrative inputs, not benchmarks, so you can see how fast small percentages add up.
Assume 400 booked appointments a month and an average ticket of $350.
No-shows at 4%: 400 × 4% = 16 missed visits. 16 × $350 = $5,600 a month, or $67,200 a year.
No-shows at 10%: 400 × 10% = 40 missed visits. 40 × $350 = $14,000 a month, or $168,000 a year.
Cancellations at 14%, half never refilled: 400 × 14% = 56 cancellations. Half unfilled is 28 visits. 28 × $350 = $9,800 a month.
Moving from 10% no-shows to 4% in this example recovers 24 visits, or $8,400 a month, before counting any rebooked cancellations.
The ticket isn't the whole cost. A missed injectable appointment still paid for the provider's time, the room and often the product prep.
Then there's the client. If she no-shows and hears nothing back, you may never see her again. That's a lifetime of visits gone, not one.
Yes, for most practices. A policy with a real consequence turns a skipped appointment into a phone call.
Associated Skin Care Professionals (ASCP) recommends putting the policy in writing with a minimum notice window, commonly 24 hours. Its example policy charges a $50 fee for late cancellations and up to 100% of the reserved service for a no-show, and requires prepayment from clients after repeated no-shows.

Here's how we set it up for medspas:
Before launch, check your payment processor's terms and your state's rules on cancellation fees. Keep the policy language plain.
The research is clear that reminders help. A Cochrane review of eight randomized trials found attendance of 67.8% with no reminders, 78.6% with text message reminders and 80.3% with phone call reminders.
Text and phone performed about the same, and two included studies found text reminders cost less per attended appointment than staff calls.

Forgetting is the main reason people miss. An American Family Physician review of text reminders cites research in which forgetting accounted for 49% of missed appointments.

The cadence we build for medspas uses both email and SMS:
| When | Channel | What it does |
|---|---|---|
| At booking | SMS and email | Confirms date, time, provider and policy. Includes a calendar link and prep instructions. |
| 72 hours before | SMS | Asks the client to reply to confirm or reschedule. A reschedule here is easy to refill. |
| 24 hours before | SMS and email | Final reminder with the cancellation window restated. Unconfirmed clients go to a staff call list. |
| 2 hours before | SMS | Address, parking and arrival time. Cuts late arrivals as well as no-shows. |
The confirmation reply is the most important piece. A reminder informs. A confirmation request commits the client and tells your front desk exactly which chairs are at risk a day ahead.
Reminders prevent no-shows. Automation recovers the ones that still happen.
Clients slip away at five moments: Get Seen, Always On, Easy to Buy, Show Up and Come Back. No-shows and cancellations live in the last two, Show Up and Come Back, and that's where a full calendar quietly turns into an empty room.
We build the system that protects them:
That work is part of our CRM, automation and AI service. For practices that want the whole system, the Practice edition Revenue System includes a website with booking and payments, reminders and no-show recovery, membership billing and reactivation, and a five-minute response standard on new inquiries.
Every engagement is scoped to your goals after a free 15-minute diagnostic.
One NYC medspa came to us with a templated site and no booking engine. Results began within 90 days, and within six months of the KamKal system build the practice saw 75% more site traffic and bookings.
Not sure where your leak is? The Revenue Recovery Audit measures all five moments against your own numbers. See how we work with aesthetics practices on our medspa marketing page. And remember that new bookings start with a strong Google Business Profile.

About 4%, according to Zenoti's 2026 benchmark report on 2025 medspa data. That figure reflects practices already using booking software, and broad healthcare research averages closer to 23% no-shows.
About 14% in 2025, per the same Zenoti benchmark data. Track late cancellations inside your policy window separately, because those are the ones that are hard to refill.
Yes, a Cochrane review found attendance rose from 67.8% with no reminders to 78.6% with text reminders. Asking clients to reply and confirm adds a commitment and shows your front desk which appointments are at risk.
Yes, most medspas should have a written fee and notice window, commonly 24 hours. Pair it with a deposit for high-value appointments or a card on file collected at booking, and enforce it consistently.
We build four touches: a confirmation at booking, a confirm-or-reschedule text 72 hours out, a reminder 24 hours out and an arrival text 2 hours before. Unconfirmed clients at 24 hours go to a staff call list.
Cost depends on scope: every KamKal engagement is quoted after a free 15-minute diagnostic based on what your practice needs. Scope is driven by your booking platform, how many providers and locations you run, and whether you need reminders alone or the full system with waitlist fill, no-show recovery and reactivation.
